Help For a Pooled Plan Provider

The Department of Labor (DOL) developed the Form PR Registration for Pooled Plan Provider to satisfy reporting requirements under ERISA and the Internal Revenue Code.

Any person who wishes to serve as a pooled plan provider must file Form PR using this website’s IFILE tool.

The How To Register as a Pooled Plan Provider publication provides an overview of how to file. For more detailed information on how to file, see the EFAST2 FAQs and tutorial videos.

Instructions and a sample form for the Form PR are available for reference.

Beginning February 1, 2021, you must submit your initial filing at least 30 days before beginning operations as a pooled plan provider.

Supplemental filings must be submitted within the later of 30 days after the calendar quarter in which specified reportable events occurred or 45 days after the actual event.

Copies of submitted filings are generally publicly available on this website’s Filing Search tool.
