• Official-government produced Form 5500 Annual Returns/Reports and Schedules for Filing - Filers who want to complete Forms 5500 or 5500-EZ for plan years between 2005 and 2008 using a pen or typewriter must use the official government-produced forms. The hand print forms, schedules and instructions can be ordered online at the IRS web site or by calling 1.800.TAX-FORM (1.800.829.3676) between 7:00am and 10:00pm Monday through Friday.

• Form 5500 Annual Return/Report Schedules and Instructions - Recent plan year forms are posted here for information purposes only. Plan year 2004 and older forms may be printed and used for filing those specific plan year returns/reports.

• Form 5500-EZ Annual Return and Instructions - Recent plan year forms are posted here for information purposes only. Plan year 2004 and older forms may be printed and used for filing those specific plan year returns/reports.

• EFAST-1 Application Form for EFAST Electronic Signature and Codes for Transmitters and Software Developers - This application is for EFAST electronic signatures only and may not be used in EFAST2.

• EFAST-1 Instructions - These instructions are for EFAST electronic signatures only and do not apply to EFAST2.

• Safe Handling Procedures for PINS and Encryption Keys - These are procedures specific to EFAST electronic credentials and do not correspond to EFAST2.
